STI Prevention Coordinator

SUMMARY

Participates in prevention services for the twenty (20) counties in the Southeast Region. Work involves travel to promote awareness to HIV infected and affected in the region, develop service plans, and to provide evidence based intervention programs, community outreach, community planning and counseling/testing to those at risk of exposure to HIV/STD's. In addition, employee assists with community awareness and education.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assist public/participants in recognizing and developing plan to alter behaviors that could be harmful to general health.
  • Schedule all participants/public appointments.
  • Maintain resource guide for services available to client/families.
  • Provide ongoing education to public/participants.
  • Coordinate information and education to public through in-services, community and school based education programs and information booths.
  • Take part in appropriate community groups and agencies that promote networking and collaboration of services.
  • Perform Orasure testing, which includes pre and post - test counseling and results.
  • Answer questions from community and provide material on disease, prevention, and care.
  • Provide DEBI programs, community outreach, and community planning for the Southeast Region (approximately 20 counties).
  • Market prevention programs through region.

RN / LPN

Position Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to support both our WIC Clinic and Public Health Clinic. This position is split between two days per week in the WIC Clinic and three days per week in the Public Health Clinic. The ideal candidate is committed to prevention, education, and improving the health of individuals, families, and the community.

Public Health Clinic Responsibilities

Provide nursing services focused on disease prevention, risk assessment and reduction, and overall health improvement for individuals and populations.

Key Duties Include:

Immunizations:

Administer vaccines and medications in accordance with protocols
Provide patient education and schedule follow-up appointments as needed
Complete accurate, timely, and confidential chart documentation

Communicable Disease Control:

Receive and review reports of communicable diseases
Conduct investigations, including patient interviews, education, contact identification, and treatment coordination
Complete and distribute required documentation to appropriate parties
Communicate effectively with internal and external stakeholders regarding public health concerns
Maintain up-to-date knowledge of communicable diseases and assist with outbreak response as needed

Family Planning Services:

Obtain preliminary patient histories and collect laboratory specimens following established protocols
Develop and deliver individualized patient education
Schedule and assist with physician visits and exams
Monitor lab results, notify patients, and provide appropriate follow-up
Administer pregnancy tests and make referrals as needed
Respond to patient concerns professionally and in a timely manner
Manage ongoing patient follow-up to ensure compliance and continuity of care

WIC Clinic Responsibilities

Support the certification and care of WIC participants in accordance with state policies and procedures. Provide nutrition and breastfeeding education while assisting with supervision of health professional assistants related to program eligibility activities.

Key Duties Include:

Collect and document demographic and health data including height/length, weight, hemoglobin/hematocrit, oral assessments, and nutrition assessments
Complete nutrition assessments at the time of certification
Identify high-risk participants and coordinate referrals to a nutritionist
Promote, support, and assess breastfeeding as the preferred infant feeding method
Determine participant eligibility and assign risk factors
Provide nutrition education and counseling for non-high-risk participants
Prescribe appropriate food packages, exempt formulas, and WIC-approved nutritionals per policy
Coordinate care with medical providers and community resources
Provide health, nutrition, and referral information to participants

Qualifications

Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
Strong communication, organizational, and documentation skills
Ability to work collaboratively in a community-based public health setting
 

Environmental Public Health Specialiast

POSITION DESCRIPTION

Summary: Work involves performing a variety of inspections in environmental health, including on-site sewage systems, consumer protection programs like restaurants, lodging places and day care settings.
Will be supervising other environmental health specialist within the department.

SALARY RANGE: $18-$20

FULL TIME - 8AM-5PM
